Sadly, though, I'll lay odds that they've managed to get the information from quite a few credit cards with that one.

I've been getting some highly amusing ones lately that say they're billing me for 'money laundering' services... "This is because you are either criminal or child pornography webmaster and have need our service".

Then the same kind of form below for if you want to contest the charges. :)
